wow you do have a mess...i think if you have feelings for this girl you have pregnant no matter whos sister this is your fixing to have a baby with her dont neglect the baby for your mistakes. its hard for me to believe you dont have some kind of feelings for her considering you have cheated on you fiance with her for a while now and to hurt someone youve been with for four years like this her sister must be pretty special the poor girl is devistated but honestly she will find someone better then you and you just need to go on with your life and be there for your baby no matter what. the family wont hate there daughter forever considering she will have there grandbaby it will get better overtime. as for you fiance she wont ever let it go but she has every right not to. good luck
